PCMH Recognition 2025-2026

TAPM Earns NCQA Patient-Centered Medical Home Recognition for 2025–2026

Greensboro, NC (November 21, 2025) – Triad Adult and Pediatric Medicine is proud to announce our renewed Recognition as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance for 2025 and 2026. This important achievement marks our ninth consecutive year of recognition and reflects TAPM’s commitment to delivering high-quality, comprehensive, and compassionate care to the communities we serve.

What PCMH Recognition Means

The Patient-Centered Medical Home model is built on a simple principle: the patient is at the center of everything we do. TAPM practices that earn PCMH Recognition must meet rigorous national standards that evaluate how well a clinic delivers coordinated, accessible, and team-based care.

This recognition highlights our dedication to:

  • Putting patients and families first

  • Coordinating services to support the whole person

  • Improving health outcomes through consistency and quality

  • Providing care that is accessible, timely, and culturally responsive

Our renewed recognition affirms that TAPM continues to meet national benchmarks for excellence in primary care.

A Team Effort Rooted in Quality

Achieving and maintaining PCMH status takes an exceptional level of teamwork and attention to detail. Every department at TAPM plays a role in this accomplishment, from front desk teams and medical assistants to clinicians, behavioral health providers, care managers, and administrative staff.

A special acknowledgment goes to our Quality Improvement and Care Management teams for leading the renewal submission and ensuring that our workflows, patient outreach, and documentation meet the highest standards.

Their work reflects the dedication of an organization that believes every patient deserves comprehensive whole-person care delivered with compassion.

Why This Matters for Our Patients

PCMH practices focus on proactive, coordinated care. For TAPM patients, this means:

  • More support in managing chronic conditions

  • Better coordination between providers, behavioral health, dental, and care management

  • Follow-up care that helps prevent complications

  • A focus on preventive services and routine screenings

  • A strong relationship with a care team that knows and understands their health needs

This recognition is not just an achievement for TAPM. It represents better health outcomes and stronger support for our patients and families.

Looking Ahead

As we enter another two years of PCMH recognition, we remain committed to continuous improvement. Our goal is to elevate the patient experience, close care gaps, expand access, and strengthen whole-person care across every TAPM practice.

Thank you to our entire TAPM team for your dedication, your resilience, and your passion for serving our community. This achievement belongs to all of us.
