Formerly Incarcerated Transition Program (FIT) Information

An individual who:

• Has been released from incarceration within the last 24 months (some exceptions can be made)
• Is a willing participant
• Is 18 years or older
• Has not been seen by a physician in over a year
• Has a chronic disease (i.e. high blood pressure, diabetes, liver disease, lung disease, kidney disease, heart disease, mental health or substance use disorders)

Individuals who have recently been released from incarceration may not qualify for Medicaid and have limited options for health insurance. The NC-FIT Program assists patients in identifying options for coverage, and removes obstacles to the cost of co-pays and medications.

Our Community Health Workers provide the following services:

• Health and Social Service navigation
• Mentorship to continue building autonomy and successfully return to the community after release.
• A bridge between clinical team and the patient
• Engaging with key stakeholders and policy makers to address community needs

The FIT program is a partnership between the following organizations:

• UNC Family Medicine
• The North Carolina Department of Public Safety
• The North Carolina Community Health Center Association
• Federally Qualified Health Centers (TAPM)
• County Departments of Public Health
• Community-based reentry organizations and local reentry councils (GCRC)
